Data on Programs and Services for Ascend Residents
The Ascend Project is a place-based initiative focused on improving outcomes in three specific neighborhoods in North Hartford: Clay Arsenal, Upper Albany, and Northeast. Program data submitted should include the following:
Residents living in ZIP Codes 06120 & 06112
Students attending: SAND Elementary School; Wish Elementary School; Milner Middle School; MLK Middle School; Weaver High School (regardless of home address)
Residents attending events hosted in Upper Albany, Clay Arsenal, or Northeast Neighborhoods and/or if your event focuses on residents of one or more of these neighborhoods

Please note that the Google Sheet version of the Program Data Request Template is read-only. To add your data, you should either make a copy in Google or email HDC staff to obtain a copy. We also offer the option to download the Program Data Request Template in Excel.
Submitting Data
Send your data files to HDC through our secure portal. Each organization will receive a private folder in which to upload their data.
